For patients on a commercial insurance plan—it is advised that patients not leave the center. For further questions, please see the Social Services Director for details. If you are leaving the center for a few hours on any other insurance, please be sure to give ample notice to your nurse so they can prepare your medications, etc. Also, please be sure to sign the notebook at the nurse’s station when you leave and return, as well as, verbally check in and out with your nurse. All patients must be back in the facility by midnight so their financial coverage will not be affected.
When the physicians, therapists and nurses determine that you are approaching your discharge goals, a representative will see you to coordinate home services, equipment, transportation, etc. A minimum of 48 hours is preferred in order to facilitate a smooth and safe discharge. Discharges are highly recommended Monday through Thursday from 9:00 am to 11:00 am.
A member of our physician services team will coordinate your case; along with the nursing and therapy staff while you are here. Our physicians round regularly, but are available 24 hours-a-day, 7 days a week.
